Calculate calorie intake to make sure that you're not starving yourself to death.  Look at your fat intake, too.  So many dieters are under the assumption that they must cut fat totally from their diet in order to lose weight.  The body must have a certain amount of fat in the diet in order to survive.
 
Also recheck your exercise routine.  Are you exercising your diet to death? What about water intake?  Are you hydrating properly.
